Inspector Notes

A monitoring inspection was initiated on May 24, 2023 concluded on June 1, 2023. Licensing Inspector was in the center on May 24, 2023 from 11:45 AM until 1:15 PM. At the time of entrance there were 30 children present, with 7 staff supervising. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during lunch, preparing for and participating in rest period and small group activities. Areas of non-compliance are identified on the violation notice.

Standard 22.1-289.035-A
Based on record review and staff interviews, the licensee did not ensure all staff shall undergo a background check every five years to include a sworn statement, criminal records check (CRC) and search of the central registry check.

Evidence: The record for Staff #1 did not include an updated CRC as the most recent one was dated 5-23-2018.
Plan of Correction: The Director scheduled a fingerprinting background check appointment for the staff member.
Standard 8VAC20-780-330-B
Based on observation and staff interviews, the licensee did not ensure that outdoor equipment with slides or climbing features shall meet requirements for resilient surfacing.

Evidence: An inflatable piece of equipment to include a slide was observed inflated and sitting directly on grass and there was no resilient surfacing. A center staff confirmed the children from Preschool Room had been playing on the inflated slide.
Plan of Correction: The manual for this structure did not require resilient surfacing; however in the future the director will ensure he structure is placed on resilient surfacing.
