Inspector Notes
An unannounced monitoring inspection was started in the center on April 10, 2024 from 10:20 AM until1:15PM and concluded on April 10, 2024. At the time of entrance,41 children were in care with 8 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed playing in centers, during story time, diaper changes and resting.
the information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Evidence: Staff #5 verified the written list of children's allergies, sensitivities and dietary restrictions for the 2 ? - 3 year old room did not include the dietary restrictions (no cows milk) for Child #2.
Evidence: Staff #4 was observed as the only person supervising 10 children in the 4-year-old room and they do not have a current certification in CPR. The certification expired in 2-2024

Evidence: The following classrooms did not have updated attendance list as required 1.There were 9 children observed in the pre-toddler room and only 8 children were on the attendance list. Center staff verified the 9th child had been in the classroom for approximately 1 hour and had not been added to the attendance list.
2. There were 4 children observed in the infant room and only 3 children were on the attendance list. Center staff verified the 4th child had been in the classroom for approximately 1 hour and had not been added to the attendance list.
