During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 4-10-2025 and completed on 4-17-2025. The onsite inspection began at 1:50pm and ended at 4:25pm. The inspector reviewed compliance in the areas listed above. There were 64 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4-24-2025. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #1 was overdue by 3 months.
Plan of Correction: The teacher will report back to work after her background check is completed.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due 21 months ago and Staff #3 and Staff #4 updated screening were due 18 months ago. The updated screenings have not yet been completed.
Plan of Correction: On 3-1-23, 9-6-23, 9-12-23 TB screenings were completed. However, the forms were completed incorrectly. Forms have been updated correctly.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. On the playground there were 2 metal pieces of equipment, a train and a composite piece that had areas of chipped and peeling paint. The was metal fence door at the entrance to the playground had a broken frame that was protruding and sharp.
Plan of Correction: The entrance gate was removed & replaced immediately
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance sheet for Room #1 listed 5 children and 6 children were present, and Room #4 listed 5 children in attendance and 7 were present.
Plan of Correction: Due to a medical emergency, children were escorted to 2 different classrooms to maintain staff/child ratios. In the future staff sign children in as they enter.
