During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on September 2, 2025. The on-site inspection began at 11:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was 12. The inspector reviewed three children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the home, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Annually, the provider shall review with the parent the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgement of the review. Records for Child #3 did not have a parent's signature for the annual review of the emergency contact information. The last review was dated 04/2024.
Repeat violation.
Plan of Correction: I will have the parent review the information today and sign to confirm the information is correct.
