During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/06/2026, as a part of the licensure period. The on-site inspection began at 9:39 am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were 6 children, 2 caregivers, and 2 household members present; the point total was 18. The inspector reviewed 5 children?s records, 2 caregiver's records, and 3 household member's records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Household Member's, over 18 years of age, must provide a sworn statement or affirmation disclosing whether they have ever been convicted of or is the subject of any pending criminal charges for any offense within or outside the Commonwealth and whether or not he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

There was no sworn disclosure in the file for Household Member #3, which was overdue by 34 days.
Plan of Correction: A sworn disclosure statement will be completed by Household Member #3 immediately and placed in her file.
