Inspector Notes

An unannounced monitoring inspection was conducted on 8/31/2023 from 9:15am to 12:45pm. At the time of the inspection there were 154 children in care and 20 staff members. Children were observed participating in floor and table top activities, story time and lunch. The record sample size was 12 staff records and 8 children?s records. Medication was also reviewed.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard 8VAC20-780-520-C
Based on observation and interviews, it was determined the center did not ensure that all of the requirements were met when diaper ointment is used.
Evidence:
1. In the Blossom?s classroom two tubes of diaper ointment were stored in a drawer that was accessible to children.
2. The licensing inspector observed a two year old child open the drawer that the diaper ointment was stored in.
Plan of Correction: Staff member immediately removed the diaper ointment from the drawer and placed it in a cabinet out of reach of children.

Based on observation and interviews, it was determined that the facility did not ensure that the center maintain a written record containing all of the required information of children's serious and minor injuries.
Evidence:
1. The injury report for Child #9 and Child #10 did not include the date and time the parent was notified of the injury.
2.The injury report for Child #11, Child #12, and Child #13 did not include the date the parent was notified of the injury.
Plan of Correction: Forms will be updated to include the date the parent was notified. Staff will ensure the time parents are notified is documented on the injury report.
