Inspector Notes

An unannounced monitoring inspection was conducted on 12/01/2023 from 10:10am to 1:45pm. At the time of the inspection there were 155 children in care and 25 staff members. Children were observed participating in floor and table top activities, outdoor play and lunch. The record sample size consisted of 6 staff records and 9 children?s records. Medication, emergency supplies, evacuation drills and required postings were also reviewed.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on observation and interviews, it was determined that the facility did not ensure all areas and equipment of the center are maintained in a safe and operable condition.
Evidence:
1.In the Rec Room a baseboard was peeled way approximately 14? from the wall and the veneer on the seat of a chair was cracked and peeling causing a scratch and splinter hazard.
2.In the Nook, the outside edge of the sisal carpet was lifted and curled up, causing a trip hazard.
3.Staff #7 (Assistant Director) confirmed the carpet in the Nook and the baseboard and chair in the Rec Room needed repairs.
Plan of Correction: The vendor was contacted to fix baseboard. The seat was removed from the room.
The carpet edge was taped down.
