Inspector Notes

An unannounced, on-site monitoring and self-reported injury inspection was initiated and completed on 8/19/24. The on-site inspection began at 9:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 222 children present and 36 staff. The inspector reviewed 8 children?s records and 6 staff records on-site. This inspection included document review tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Repeat Violation
Based on observation and interviews, it was determined the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
1.On the preschool playground there was a hole, approximately 8 to 10 inches deep, in the grass beside the sandbox. This hole created a tripping hazard.
2.In the School Age Club room there was a hole in the wall that contained exposed wires and a screw. The hole was low on the wall in an area accessible to children.
3.There was a split wooden board in the Construction Lab.
4.In the older infant play area, there was chipped paint and exposed dry wall in an accessible area to children.
5.In the Older Two?s classroom, the corner of an art easel was broken.
6.Staff #7 confirmed these areas were in need of repair.
Plan of Correction: The areas in need of repair including the hole in the wall, split board and chipped paint have been fixed. The hole on the playground is covered and the repair is being assessed.
Standard 8VAC20-780-280-B
Based on observation and interviews, it was determined that the facility did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1.There were cans of shaving cream labeled ?keep out of reach of children? in unlocked and accessible areas of the classrooms.
a.In the art room there were 14 cans of shaving cream in a cubby.
b.In the PreK Two?s classroom there was a can of shaving cream in an unlocked cabinet in the bathroom.
c.Staff #7 confirmed the cans were not locked up.
Plan of Correction: Shaving cream has been placed in locked cabinets.

Based on observation and interviews, it was determined the facility did not ensure that hazardous substances, such as cleaning materials, not kept in the original container were clearly labeled to indicate the contents.
Evidence:
1.There were unlabeled spray bottles in the lab and stem classrooms.
a.Staff #7 confirmed the bottles were not labeled.
Plan of Correction: All spray bottles have been labeled.
Standard 8VAC20-780-560-G
Based on observation and interviews, it was determined the facility did not ensure that when food is brought from home it is clearly dated and label in a way that identifies the owner.
1.A lunch box in the older three-year-old classroom was not labeled with a name or date.
2.On the school age playground there were 3 water bottles that were not labeled with dates or names.
3.Staff #7 confirmed the names and date were not on these items.
Plan of Correction: An announcement went out to parents reminding them to label lunchboxes and water bottles. Labeling stations have been created in classrooms for parents.
