During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 04/14/2025. The on-site inspection began at 1:36 pm and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 5. The inspector reviewed 2 children?s records and 3 caregiver/household member(s) records on-site.

This inspection included document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/28/2025. A POC submitted after this date will not appear on the public website.

This inspection was amended on 04/21/25 to change the inspection type to "renewal".

Providers are required to have employees and household members undergo a background check every five years.

The most recent central registry check for household member #1 was overdue by 1 month.
Plan of Correction: Moving forward I will make sure to have a reminder in my phone and calendar to check folders of household members as well as employees.

Children under two years of age shall not use pillows or filled comforters.

Child #1, age 16 months, was asleep in the crib. There was a pillow in the crib with the child.
Plan of Correction: I will make sure to not have any pillows or any toys in cribs/beds.
