Inspector Notes
A mandated monitoring inspection was conducted on December 5, 2023 from 10:40am-11:20am. There were 7 children (17points) in care with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing with puzzles and infants and toddlers were given an opportunity to play and explore. A total of 2 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

5Evidence: There was no attendance taken on 12/5/2023.
Evidence: Child 1 is missing the information to parents form which includes check in check out procedures, termination policy, discipline policy and more.
Evidence: Child 1 (start 8/8/23) and 2 (start 8/14/23) do not have documentation that the provider viewed an original proof of identity.
Evidence: There was no drill documented for November 2023.
Evidence: There was no posted menu.
