Inspector Notes

A mandated renewal inspection was conducted on May 30, 2024 from approximately 4:05pm-5:00pm. There were 8 children in care with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 household member record and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on July 23, 2024. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.
