During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was completed on 11/13/2024, as a part of the licensure period. The on-site inspection began at 11:40 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 4 caregivers; the point total was 23 points. The inspector reviewed 5 children?s records and 5 caregiver/household members records.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/26/2024. A POC submitted after this date will not appear on the public website.
Violations
4The findings from the most recent inspection, dated 05/30/2024, were not posted in a conspicuous place on the premises.
Caregiver #2 had been employed for five months, before receiving a completed national criminal background check. Caregiver #4 had been employed for 6 days prior to receiving a completed national criminal background check.
Staff #2, who had been employed for six months, did not have a central registry search results and contact was not made to check on the results until October 1.
The provider did not have an evacuation drill practiced in the month of October 2024.
