Inspector Notes
A mandated monitoring inspection was conducted on 12/15/2023 from 10:20am-11:00am. There was 1 child in care with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The child in care recited their letters, traced in a notebook, sang songs and played freely with toys. When the child asked to play outside, the provider replied they would go outside after the inspection was completed. A total of 1 child records,1 staff record and 2 household members were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence: Household member 1 has a fingerprint based background check which was completed on 8/2/2018.
Evidence: Attendance has not been taken since April 2023.
Evidence: There was no emergency evacuation drill documented for November 2023. All previous evacuation drills were conducted between the hours of 10:00am and 11:00am.
