During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on 12/04/2024. The on-site inspection began at 10:25 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 19 pts. The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/11/2024. A POC submitted after this date will not appear on the public website.

A person 14 years of age and older who is living in the home must have a search of the central registry.

Household member #1 is over 14 years old and did not have a search of the central registry on record.
Plan of Correction: We are already working on it.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Child #3 was attending the daycare for 11 months without a physical exam on record.
Plan of Correction: They out from the daycare. They are going to bring the papers when they return.
Standard 8VAC20-800-530-B
Cribs when being used by infants shall have a tight-fitting bottom cover.

2 of 2 cribs had a bottom cover that had an excess of approximately 8 - 12 inches of fabric.
Plan of Correction: We use appropriate cover right now.

A child's record shall contain the following information: address and telephone number of each custodial parents place of employment.

Child #3 did not have the address and phone number listed for one custodial parent.
