Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/13/2021 and concluded on 04/13/2021. The provider was contacted by telephone to initiate the inspection and the inspection was completed via FaceTime. There were 7 children present (10 points) with the provider and one staff. The Inspector reviewed 2 children?s records and 3 staff/household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the provider did not review the emergency plan at least annually and update the plan as needed.

Evidence:
1. The last documented review of the emergency plan was dated 12/19/2019.

Based on record review, the provider did not ensure all adults living in the family day home completed a sworn statement or affirmation every 5 years as required.

Evidence:
1. The sworn statement in the record for Household Member #1 was dated 01/20/2016.
2. The sworn statement in the record for Household Member #2 was dated 01/20/2016.
3. The sworn statement in the record for Household Member #3 was dated 02/01/2016.
