Violations

1
Standard 22.1-289.036-A
Based on record review, fingerprint based criminal history record checks were not completed every five years as required for the provider and two household members.

Evidence:
The fingerprint-based criminal history record check results in the record for Caregiver #1, Household member #1, and household member #2 were dated 09/14/2018.
Plan of Correction: Upon learning the backgrounds were expired, the provider attempted to schedule appointments last week to complete the fingerprint checks. The appointments were not able to be completed. The provider will follow up to confirm their code required to schedule the appointment is correct and provide documentation that appointments have been scheduled within 10 business days.
