Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/12/2024, as a part of the licensure period. The on-site inspection began at 10:35am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 16. The inspector reviewed 3 children?s records and 4 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The family day home is required to obtain a completed sworn statement prior to the employee's first day of employment.

Evidence:
Caregiver #1 who had been employed for seven months, did not have a completed sworn statement.
Plan of Correction: A sworn statement will be completed by Caregiver #1.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Evidence:
The fingerprint check results for Caregiver #1 were obtained 15 days after Caregiver #1's first day of employment.
Plan of Correction: A fingerprint check was completed for Caregiver #1. Fingerprint checks will be completed before date of hire.

Providers must request a search of the central registry prior to the employee's first day of employment.

Evidence:
Caregiver #1, who had been employed for seven months, did not have a central registry search requested.
Plan of Correction: An request for a search of the Virginia Central Registry will be requested for Caregiver #1.
Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver that contains documentation that two or more references as to character and reputation as well as competency were checked before employment.

One reference in the record for Caregiver #1 was obtained one day after Caregiver #1's first date of employment. A second required reference was not obtained.
Plan of Correction: References will be obtained prior to date of hire. Provider will obtain an additional reference for Caregiver #1.

Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.

Evidence:
The tuberculosis screening for Caregiver #1 was obtained 9 days after Caregiver #1's date of hire.
Plan of Correction: Tuberculosis screening results will be obtained before date of hire.

The family day home is required to practice the emergency evacuation procedures monthly with all
caregivers and children in care during all shifts that children are in care.

The family day home did not practice emergency evacuation procedures in August 2024.
Plan of Correction: An additional make-up evacuation drill will be practiced.
