During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/20/2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 16. The inspector reviewed 5 children?s records and 4 caregiver/household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-1
Providers are required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Caregiver #2, who has been working for less than a week, was not dated.
Plan of Correction: Caregiver will add the completion date to the form.
Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver that contains all required information. The record for Caregiver #2 did not contain verification of age and the date and signature for one reference.
Plan of Correction: The missing information will be added to the file.
Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. There was no tuberculosis screening for Caregiver 2, who has been caring for children for less than a week.
Plan of Correction: The caregiver is scheduled to return for the reading today.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by six months. The TB screening in the record for Household Member #1 was obtained more than two months past due.
Plan of Correction: Caregiver will schedule an appointment.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunizations is to be obtained for each child. The immunizations for Child #2 were dated two weeks after Child #2's first day.
Plan of Correction: Provider will get immunizations from parents before the child's first day.
