Inspector Notes
An unannounced monitoring inspection was initiated on 10/15/2024 and completed on 10/15/2024 with a local agency in response to a self-report received by the licensing office on 10/09/2024 relating to forbidden actions. The inspector reviewed 1 child's record and 1 staff record on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration, staff qualifications and training and programs and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/08/2024. A POC submitted after this date will not appear on the public website.
Violations
7There is no documentation of a physical exam on file for child 1 who has been in care for 12 months.
Staff 1 has not completed The Virginia Department of Education-sponsored orientation course. Staff 1 has been working at the facility for more than 90 days.
On October 8, 2024, child #2 ( age 44 months) was restrained in a highchair with no meal, no activity, toy or attention by staff #1 for approximately 33 minutes. Staff #1 states she routinely places child #2 in a highchair due to his disruption, and she did not know of any other way to keep him from misbehaving.
Staff #1, the only staff present in the preschool classroom, on the morning of October 8, 2024, does not have written documentation on file to demonstrate that she meets program leader qualifications.
When the inspector entered the facility on October 15, 2024 at approximately 11:00 am, staff 3 was speaking to children, in one preschool classroom, using a tone of voice that was irritated, negative and did not provide comfort to the children or positive re-direction.
On October 8, 2024, staff #1 picked up child #1 ( age 3 years) by the lower part of her left arm and her feet left the floor. Child #1 was crying and injured. As a result, child #1 required medical attention at the Emergency Room.
All staff have been retrained on forbidden actions and handling of children. Staff will continue to get reminders.
On October 8, 2024, Child #2 ( age 44 months) was restrained in a highchair for approximately 33 minutes not during mealtime.
