During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on May 19, 2025, and completed on May 21, 2025, in response a self-report received by the licensing office on May 19, 2025, relating to allegations of supervision and transportation. The inspector reviewed one child record and one staff record on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 9, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-F
Children under 10 years of age shall be within actual sight and sound supervision of staff. On May 16, 2025, a 7-year-old was left alone for approximately 20 minutes on the center?s transportation vehicle without staff supervision.
Plan of Correction: New check sheets are given to van drivers and supervisor will check behind all van drivers.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, and services are maintained in compliance with these standards, and the center's own policies and procedures that are required by these standards. Per the center?s supervision policy, ?Children shall be supervised at all times.? Per the center?s transportation policy, ?The van driver will ensure that all children are off the van.? After Staff #1 parked the van at the center, Child #1 was left on the transportation vehicle alone for approximately 20 minutes without adult supervision.
Plan of Correction: Van driver no longer employed. All staff are being retrained on all safety aspects of the center including van safety and policies.
Standard 8VAC20-780-580-C
At least one staff member or the driver always remains in the vehicle when children are present. On the way back to center, Staff #1 went into a store and left Child #1 in the vehicle alone and unsupervised. No other staff members were in the vehicle.
Plan of Correction: A vehicle and car safety class will be attended.
Standard 8VAC20-780-580-K
Staff shall verify that all children have been removed from the vehicle at the conclusion of any trip. Staff #1 never verified that all children had exited the transportation vehicle. Child #1, age 7 years old, was left alone for unattended in the center?s transportation vehicle without staff supervision for approximately 20 minutes.
Plan of Correction: All staff are being retrained on all safety aspects of the center including van safety and policies.
