The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1 did not have a copy of his immunization records and has been enrolled at the center for two months.
Plan of Correction: The center?s plan of correction: During the time of the inspection, the center Director called the parent, and the medical documents will be obtained and provided to the center tomorrow.

Each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Child #1 did not have a copy of his physical examination and has been enrolled at the center for two months.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. On the outdoor playground, the plastic basketball hoop did not have a stable base and poses as a tipping hazard. The toddler room has peeling paint on the walls and dried paint chips were observed on the floor.
Plan of Correction: The center?s plan of correction: The center will have the basketball hoop removed from the playground at this time and will add the appropriate weight to the bottom to keep the base stable when children are playing with it. A work order will be put in for the peeling paint and it will be painted this weekend.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In the two-year-old classroom, there was a cleaner attached to a mop that has a warning label, ?Keep out of Reach of Children? and it was unlocked accessible to children in care.
Plan of Correction: The center?s plan of correction: During the time of the inspection, the cleaner was locked up. The director will reinforce to all staff that all cleaning materials must be locked when not in use.
