During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 10, 2026. The on-site inspection began at 10:15am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 only had the initial immunization records from enrollment and no updated records.
Plan of Correction: The center responded with the following: The center will obtain the updated immunization records for Child #1 and will audit files to ensure all immunizations for children under the age of 2 years old are up to date.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The storage unit on the children?s playground had frayed edges of wood to the storage doors. Children were observed playing close to the storage which poses as an injury/splinter hazard.
Plan of Correction: The center responded with the following: The administrator will have the doors of the storage reinforced and repaired.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Staff had 18 children in the same playground area from toddler age to 5 years old. Several older children, ages 4 and 5 years old, were observed running and bumping into toddlers knocking them down.
Plan of Correction: The center responded with the following: Administration was prompted and they immediately had the children come into the center. Administration has two designated areas for older children and infant/toddler?s on the playground however it was not being used separately. They will use the appropriate area playground. Older children and young toddler?s will not be on the same playground at the same time. Classes will be staggered when using the playground. Staff will be re-trained in playground safety.
