During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/13/2025 and completed on 5/13/2025. The on-site inspection began at 10:51 am and ended at 12:32 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and two staff. The inspector reviewed five children?s records and two staff records on-site on 5/13/2025.

This inspection included document review, tour of the facility, interviews and observations.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected.

Submit your POC within five (5) business days from today, which will be the close of business on 5/28/2025. A POC submitted after this date will not appear on the public website.

A daily simple health screening and exclusion of sick children by a person trained to perform such screenings.

Staff #1?s daily health observation was expired and overdue by seven years. There are no other staff currently. trained in daily health observation.
Plan of Correction: Both preschool teacher will participate in a virtual Daily Health Observation course at the end of this preschool year (after June 5, 2025)

The center is required to have employees undergo a background check every five years.

The most recent central registry check for Staff #1 was overdue by two years, and for Staff #2 overdue by nine months. The National fingerprint background check and sworn disclosure statement were overdue by eight months for Staff #1 and Staff #2.
Plan of Correction: The two preschool teachers and the prek director will obtain new fingerprints and background checks at the end of this preschool year (after June 5, 2024)
