Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 3/25/2021 and concluded on 3/26/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records, 2 staff records, and 5 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the family day home did not obtain documentation of additional immunizations every six months for children under the age of two years.

Evidence: Child #1's most recent immunizations on file were dated 6/15/2020. Child #3's most recent immunizations on file were dated 7/31/2020. Child #1 and Child #3 are both under the age of two years.
Plan of Correction: I will tell their parents to bring in their shot records.
