Inspector Notes

An unannounced monitoring inspection was conducted on 11/28/2023 from 12:55pm to 1:55pm. There were 11 children ages 3 months to age 4 years old (30 points) with the Provider and an assistant. The programming area, attendance, menus, Provider, Assistant and Household member records, 5 children?s records, emergency drills, emergency supplies, and policies were reviewed. Outdoor area was not inspected due to the weather. There were no medications onsite. Children were observed napping and feeding. There was an adequate number of staff present with current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Based on review of 5 child records, the provider did not ensure they obtained documentation that children were adequately immunized prior to attending.

Evidence: The record for Child #4 (start date 7/18/23) did not contain immunizations prior to attending.
Plan of Correction: I'll make sure to get the child's missing documentation from the parents to put on file.
