Inspector Notes

An unannounced monitoring inspection was initiated and completed on 03/24/2023. The inspector was onsite from 10:40am until 12:30pm
.
There was 1 children, age 2 years old present during the inspection. The child was observed during free play and a time.

The Inspector reviewed 2 children's file, one staff file and one household member file.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the provider failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.

Evidence: Child #1 has an enrolled on 1/24/2023, had no documentation of a physical on file during the inspection on 03/24/2023.
Plan of Correction: Parent will have until 4/7 to get physical or child can not return.
