Inspector Notes

An unannounced inspection was initiated on 6/25/2024 and concluded on 09/11/2024. In response to a complaint received by the licensing office on 6/21/2024 regarding allegations of physical plant, administration, forbidden action, and handwashing. The inspector and administrator were on site from 7:30am until 8:50am. The inspection included observations, interviews, and record review.

The preponderance of evidence gathered during the investigation supports some of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/18/2024. A POC submitted after this date will not appear on the public website.

The center shall afford the Department reasonable opportunity to inspect all of their facilities, books and records. The Department requested access to view a video recording as a part of an inspection of records on three occasions. The center administrator did not provide opportunity to inspect this record.

Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, centers must obtain the results of a central registry search for an employee, within 30 days of employment. Staff 1 did not have central registry search results during the inspection and had been employed for approximately 2 months.

Each staff member shall submit documentation of a negative tuberculosis (TB) screening signed by a physician, physician designee, or an official of the local health department. Staff 1, who has been employed for approximately two months, did not have a TB screening on file signed by a physician, physician designee, or an official of the local health department.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. In the infant room, there was gray duct tape covering the corner of the bottom wall. The tape had been pulled apart in some areas and cement and drywall were showing. The paint was also chipping away and was accessible to the children.

REPEAT VIOLATION
Children?s hands shall be washed with soap and running water after eating meals and staff shall wash their hands with soap and running water or a disposable wipe after any contact with body fluids. In the two-year-old room, a child was observed eating and did not wash their hands or use a disposable wipe afterward. A staff person was observed picking up child clothing from the floor in the bathroom. The staff person stated a child had a toileting accident. The staff person moved the soiled clothing and did not wash their hands after the contact.

REPEAT VIOLATION
Diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the system during disposal. The diapering pail in the two-year-old room was not operable by foot or self-opening. Staff in the classroom stated they have to open the trashcan by hand.
