During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/5/2024, as part of the licensure period. The on-site inspection began at 8:15am and ended at 10:30am. The inspector and administrator reviewed compliance in the areas listed above. There were 51 children present and 14 staff. The inspector reviewed 5 children?s records and 4 staff record were reviewed on-site.

This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/17/2024. A POC submitted after this date will not appear on the public website.

Repeat Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the toddler room, bottles of sanitizer were on the top of the sink in the classroom. These were accessible to the children. In the bathroom of the toddler room, there were bottles of toilet cleaner in the cabinet. These chemicals were unlocked.

If rest mats are used, they shall have cushioning and be sanitized on all sides weekly or before use by another child.

In classroom 2,3 and 4 the mats were located in the bathroom. The mats had spit on the sides and the foam was exposed.

Repeat Violation
Diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the system during disposal.

In the small 3-6 year old room?s bathroom, there were diapers in a trashcan with no lid. The teacher said she did not have a foot operated diapering system.
