During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 04/30/2025, as part of the licensure period. The inspectors conducted the on-site inspection starting at 9:05am and ending at 11:45am. The inspector reviewed compliance in the areas listed above. There were 69 children present and 13 staff. The inspectors reviewed 5 children?s records, one medical record and 4 staff records on-site.This inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/12/2025. A POC submitted after this date will not appear on the public website.
Violations
6The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #4, who had been attending for six weeks, did not have documentation of an immunization record on file.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
Child 1, who had been attending for 2 months, did not have documentation of a physical on file.
Child 4, who had been attending for 6 weeks, did not have documentation of a physical on file.
Staff 1 updated screening was due 11 months ago. The updated screenings have not yet been completed.
The record for Child 4, enrolled for 6 weeks, was missing the following required components: proof of child?s identity and age.
Staff #3, who has been actively working at the center for the past month, did not have any completed reference verifications on file.
Child 4 had no record of these written agreements at the time of the inspection.
