Inspector Notes
An unannounced monitoring inspection was conducted on 8/22/2022. There were 12 children, ages 3 years-4 years and 2 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed. The children were observed during active time on the playground, circle time and lunch. The inspector arrived for the inspection at 9:45am and departed the center at 11:45 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
3Evidence: Staff #3 was hired on 2/21/2022 and on the day of the inspection did not have the required sworn disclosure statement.
Evidence: Child #2 first date of attendance was 8/8/2022 and on the day of the inspection did not have the required immunization record.
Evidence: Observed 2 pieces of broken and inoperable equipment on the playground.
1. The bolt on the top bar of the front seat on the blue and red airplane see saw play structure was protruding approximately 2 inches causing an entanglement hazard. There were no children observed on the see saw structure but it was accessible to the children.
2. The blue, green, red and purple caterpillar tube structure was cracked along the top portion causing a pinch point. There were no children observed in the caterpillar play structure but it was accessible to the children.
