Inspector Notes

An unannounced monitoring inspection was conducted on 9/25/2023. There were 11 children, ages 2 years-4 years, 1 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 1 staff record were reviewed. The children were observed during active time on the playground, handwashing, lunch and nap. The inspector arrived for the inspection at 10:50am and departed the center at 12:15 pm.

Discussion on standards related to emergency procedures-continuity of care.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and discussion, the center failed to ensure that the required amount of resilient surfacing was maintained throughout the playground.

Evidence:
1 Licensing staff observed approximately 4-6 inches of resilient surfacing around the fall zones of the 4 foot tall yellow and blue climbing structure.
2 Licensing staff observed approximately 3 inches of resilient surfacing and exposed tree roots around the fall zones of the 3 foot tall yellow and red climbing structure.
Plan of Correction: Provider will ensure resilient surfacing meet required depth.
