Inspector Notes

An unannounced renewal inspection was conducted on 2/5/2024. There were 12 children, ages 3 years-5 years, 2 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 2 staff records were reviewed. The children were observed during center time, circle time and active time on the playground. The inspector arrived for the inspection at 2:30pm and departed the center at 4:00 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on the review of three children?s records, the center failed to ensure that each report shall be signed by a physician, his designee, or an official of the local health department.

Evidence:
1. Child #3 start date was 8/7/2023 and on the day of the inspection the immunization and physical record was not signed by a physician.
Plan of Correction: Provider will obtain a physician signature for the physical and immunization record.
