Inspector Notes
An unannounced inspection was conducted on 9/22/22. The provider and one assistant were present with 11 children, 22 points. The children are busy playing, the provider has her childcare set up for morning preschool. The children are playing in centers and transition time, snack and large motor skill play was observed. The inspector reviewed 2 staff files and 4 children's files as well as medications and all other standards. Lots of interaction between the caregivers and children.
A violation notice was presented to the provider along with his summary.

3Evidence: Based on the review of 4 children's enrollment records, one child, child 4 is missing the emergency contacts. The emergency contact currently listed is the father.
Evidence: One over the counter medication for child 5, does not have the medication authorization.
Evidence: Based on the review of prescription medication for the children, one medication, for child 6, is not in the original box with the prescription label.
