An unannounced, on-site monitoring inspection was initiated on 09/09/24 and concluded on the same day., as a part of licensure period. The on-site inspection began at 10:00 and ended at 11:00. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregiver; the point total was 14. The inspector reviewed 3 children?s records and 2 staff records . Based on the review of children's records, the provider failed to have the liability insurance disclosure paperwork.
Evidence: The three children's files reviewed did not have the form informing parents of the providers liability insurance.
Plan of Correction: I will send the information home with the children today

The provider failed to have an emergency preparedness plan available.
Evidence: During the inspection the provider could not locate the emergency preparedness plan, only the one page plan for the parents could be located.
Plan of Correction: I know I have this somewhere, I will locate it or fill out a new form.
