Staff #1, employed for six months and 28 days, did not have documentation the central registry was requested prior to employment. The results of the central registry had not been obtained as of December 16, 2025. Staff #1 was observed working alongside two staff members.

At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening. 1. The most recent TB screening for staff #2 was dated 08/26/2023. An updated tuberculosis screen was due three months ago. 2. The most recent TB screening for staff #7 was dated 04/28/2023. An updated tuberculosis screen was due seven months ago.

Bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location. Child #4 was observed drinking a bottle while lying in his crib. A second infant was observed on the mat with a bottle propped using a bottle propping pillow.
Plan of Correction: Per the administrator: We will go over risk of bottles in beds and propping bottles. They will not be propped any more.
