During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted in conjunction with the Code Monitoring Inspection. Subsidy standards that are not covered in Code standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliances with applicable Subsidy standards, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August
29, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-580-B
The vendor shall obtain documentation of additional immunizations once every six months for children less than two years of age.

Immunizations for Child #1 were not completed every 6 months.
Immunizations for Child #3 were not completed every 6 months.
Immunizations for Child #5 were not completed every 6 months.
Plan of Correction: Notes were sent out to the parents requesting up to date shot records.
Standard 8VAC20-790-600-D-1
All staff who work directly with children shall have within 90 days of the date of employment.

Staff #1 has been employed for more than 9 months has not completed CPR and First Aid training.
Staff #3 has been employed for more than 10 months has not completed CPR and First Aid training.
Plan of Correction: Both staff members will be signed up to complete CPR and First Aid training.

There shall be at least 12 inches of space between occupied cribs, cots, beds and rest mats.

In the infant room located in building #2, two infants were observed sleeping in cribs that did not have at least 12 inches of space between the two cribs.
Plan of Correction: The teacher moved the crib after nap time.
