Inspector Notes

A renewal inspection was conducted on January 10, 2023. There were four children present, ranging in ages from two years to five years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The provider stated there were no medications on site and transportation is not provided. A total of four child records and two staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Based on a review of children's records, the provider did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
The records for child #1 (start date: 06/09/22), child #2 (start date 06/09/22), and child #4 (start date: 12/18/21) did not have a physical examination.

Based on observation, the provider failed to cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers.
