Inspector Notes

An unannounced monitoring inspection was conducted on 9/27/2022. There were 29 children, ages 5 months ? 4 years and 6 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 4 staff records were reviewed. The children were observed during active time on the playground and circle time. The inspector arrived for the inspection at 9:54am and departed the center at 11:48am.

Discussion on standards related to playground area.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on the review of four staff?s records, the center failed to ensure that a follow up tuberculosis screening for staff members was obtained.

Evidence: Staff #3 was hired on 02/17/2019 and on the day of the inspection the most recent tuberculosis screening expired 7/22/2022.
Plan of Correction: Provider will schedule a tuberculosis screening for staff.

Based on observation, the center failed to ensure that diapering surfaces shall be cleaned with soap and water and sanitized after each use.

Evidence: Observed orange cracker crumbs on the diapering surface in the 15 month old room. There was no observation of cleaning and sanitizing the diapering surface prior to an observed diaper change.
Plan of Correction: Provider will ensure staff are cleaning and sanitizing diapering surface prior to all diaper changes.
