Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 12, 2021 and concluded on April 19, 2021. A self-reported incident was received by the department regarding allegations in the areas of forbidden actions. The in-charge person was contacted by telephone to conduct the investigation. The inspector emailed a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.

Based on interviews, the center failed to ensure that staff did not use physical punishment or strike a child as punishment.

Evidence:
1. Staff #1 stated that she "popped" a six year old child on the head on April 12, 2021 while the child was in the play land area. This happened after 3:00 pm when the staff person was not "on the clock" supervising the children. Staff #1 did state that she had orientation on forbidden actions and the discipline policies of the center at hire on March 17, 2021.

2. A collateral witness stated that they observed staff #1 strike the six year old child in the area of the face in a plucking or flicking motion.
Plan of Correction: Staff #1 was terminated immediately after the incident happened on 4/12/2021.
