Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on April 15,2021 and concluded on May 4, 2021 . A complaint was received by the department regarding allegations in the areas of supervision, injury reporting, and COVID protocols. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations related to the complaint identified during the course of the investigation can be found on the violation notice.
Violations
2Evidence:
1. Interviews revealed that a child notified a staff person of an injury during the after school program on 4/13/2021. The parent was not notified on 4/13/2021 of the injury to the child.
Evidence:
1. An injury report was completed on 4/14/2021 for an injury that happened to a seven year old child on 4/13/2021. The report included several inconsistencies with the circumstances surrounding the injury. The following was documented inaccurately on the injury report: the date of the injury was documented as 4/14/2021 when the injury occurred on 4/13/2021; the information for treatment given stated the injury was cleaned, but upon interviews there was no first aid treatment given; pictures of the injury and interviews revealed three round abrasion marks on the child's lower back, but the injury report documented the injury type as a bruise to the child's lower back; interviews revealed that the injury was discussed with the parent the morning after the incident, but documentation on the injury report states that there was discussion with the parent about the injury on 4/15/2021.
