Inspector Notes
A complaint inspection was initiated on 11/10/2021 and concluded on 01/12/2022 . A complaint was received by the department regarding allegations in the areas of supervision. The Licensing inspection conducted telephone and IN person interviews.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations related to the complaint identified during the course of the investigation can be found on the violation notice.

Based on observation, interviews, and record review, the center failed to ensure that all children assigned to a different age grouping had the required items.
Evidence:
1. Child # 1 date of birth of 01/08/2021 entered care on 09/27/2021 and placed in the two year old room. In the record was a note from the parent stating it was ok to move the child to the older classroom. The record was missing a written assessment by the program director and program leader.
2. Child # 2 date of birth was 10/08/2019 and on 11/10/2021 the child was moved to the 4 year old classroom. There was no documentation in the child's record that gave permission for the child to be moved to an older grouping of children. The parent was aware of the move to the older class to help the child in a behavioral situation.

Plan of Correction: In the future the center will obtain all documentation for children if appropriate to be moved to the older classroom. This will only happen when children are ready to be in that age grouping.
