Inspector Notes

An unannounced on-site monitoring inspection was conducted on 05/31/2023. There were 131 children, ages 2 months-12 years, and 17 staff members present during the inspection. The inspector observed the following: free choice activities, lunch and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 12 15 PM and departed at 4:00 PM.

Repeat Violation

Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.

Evidence:

In the cabinet under the sink in the creepers room there were several chemicals stored in a basket. The cabinet door?s locking mechanism did not prevent entry to the chemicals and was accessible to the children.
Plan of Correction: Staff will make sure that the lock is working and or will replace as needed.

Based on observation, the center failed to ensure that all chemicals were not stored with or above food.

Evidence:

In the creepers room a large container of infant formula was stored in a basket on the same shelf as the unlocked chemicals.
Plan of Correction: The infant formula will be stored in a separate areas away from chemicals.

REPEAT VIOLATION

Based on review of the attendance and interviews with staff, the center failed to ensure that for each grouping of children, the center maintained a written record of daily attendance that documented the arrival and departure of each child in care as it occurred.

Evidence:

At 12:31 PM the written attendance for the creepers room was not completed for the day of inspection. Four children were in attendance that day.
Plan of Correction: The lead teachers will make sure that attendance is completed as children arrive and depart.
