Inspector Notes

An unannounced on-site monitoring inspection was conducted on 11/29/2023. There were 82 children, ages 3 months-5 years, and 21 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play in door play area, and lunch. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: supervision in the indoor play area. The inspector arrived at 9 AM and departed at 11:30 AM.

Based on observation, the center failed to ensure that all toileting requirements were met.

Evidence:

Observed urine in a toilet chair in the 3-year-old bathroom. All toilet chairs shall be emptied promptly and cleaned and sanitized after each use.

Based on review of the attendance and interviews with staff, the center failed to ensure that for each grouping of children, the center maintained a written record of daily attendance that documented the arrival and departure of each child in care as it occurred.

Evidence:
At approximately 9 AM the attendance for the Creeper?s room only had one child marked as present. There were four children present.
