During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/28/25 and completed on 08/28/25. The on-site inspection began at 9:15am and ended at 10:40am. The inspector reviewed compliance in the areas listed above. There were 32 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-I
The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center administers only those that were dispensed from a pharmacy and maintained in the original labeled container with the prescription label attached.

The emergency medication for child A was not in the original pharmacy labeled box as required.
Plan of Correction: The parent will be notified that the original pharmacy labeled box needs to be provided or that a separate pharmacy label be provided.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

The authorization to administer the emergency medication to child A had expired and not renewed as required.
Plan of Correction: The parents will complete a new authorization while awaiting the long term use authorization from the physician. The parents will complete new authorizations every 10 days until the long term use authorization is received.
