Inspector Notes

An unannounced monitoring inspection was conducted from 2:30 - 3:00 pm with staff. There were 19 children, ranging from five years to 10 years of age, present with two staff supervising. Children were observed having free play and participating in a group activity making paper airplanes. Two (onsite) staff records were reviewed at the time of the inspection, along with one medication, the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary preferences, and fire and health inspections. Two child records and two staff records were reviewed at the Massad YMCA on 8/31/2022 and 9/12/2022. Based on review of two staff records, and an interview with staff, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: The record for staff #2 (Date of hire/rehire: 10/11/21), did not have documentation of a Central Registry search report on file.
Plan of Correction: A Central Registry search request was mailed on 8/24/22.

Based on review of the written attendance record, and an interview with staff, it was determined that the facility did not ensure all children were included on the written attendance record. Evidence: 19 children were observed, however only 13 children were included on the attendance sheet.
Plan of Correction: Will retrain staff to ensure children are accurately marked in/out as they enter/exit the program.
