Inspector Notes
An unannounced renewal inspection was conducted from 2:00 - 2:45 pm with staff. There were 31 children, ranging from 5 years to 11 years of age, present with 2 staff supervising. Children were observed having free play and completing homework. Two children's records and two staff records were reviewed. There was no medication to review, however both staff have current certification in Medication Administration Training (MAT), first aid and CPR. The emergency drill log, first aid kit, and the battery operated radio and flashlight were reviewed and in compliance. Based on review of two child records, not all children had documentation of a physical examination by or under the direction of a physician. Evidence: the record for child #2 (date of attendance: 8/2021) did not have documentation of a physical examination.
Plan of Correction: Will request parent submit missing documentation.
Standard 8VAC20-780-560-F
Based on documentation review and an interview with staff, a menu listing foods to be served for snacks was not dated for the current week. Evidence: the menu listed dates that were repeated from a previous month and the day/date did not line up with the current day/date or with what was actually being served.
Plan of Correction: The menu was updated to reflect the actual dates for the week.

Based on review of two child records, it was determined that not all records contained all required information. Evidence: the record for child #1 did not have the name, address or phone number for a second emergency contact.
Plan of Correction: Will obtain the missing information.
