During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 24, 2025, as a part of. The on-site inspection began at 10:55am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 63 children present and 13 staff. The inspector reviewed six children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The program did not have the license, current inspection notice, emergency numbers and evacuation map, and daily schedule posted.
Plan of Correction: Make sure parent board is posted on premises in secured location.
Standard 8VAC20-780-540-E
Nonmedical emergency supplies shall be required.

The center was not able to demonstrate a working battery-operated radio.
Plan of Correction: Center will have working battery-operated radio with batteries.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.

Child #1's record did not contain documentation of the mother's place of employment and work phone number, and two emergency contacts.
Plan of Correction: Ensure all documentation is completed.
