Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on September 18, 2023. There were 3 children ages 2 1/2 to 3 years old present under the direct care of 1staff. The Licensing Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Center failed to ensure that all children in the center are in compliance with the immunizations of children against certain diseases.

EVIDENCE:
Record for Child #1 did not include documentation of the child's immunizations.
Record for child #3 did not include documentation of the child's immunizations.
Plan of Correction: Center Director will have parents submit/review immunizations.

Based on discussion with the Center Director, the Center was not equipped with a Carbon Monoxide detector in each building built before 2015 and houses a child day program serving preschool age children.
