Inspector Notes
An unannounced monitoring inspection was conducted today at 11:20am. At the beginning pf the inspection there were 4 children directly supervised by 1 staff. The home, 3 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

1Evidence:
Child 4's record did not contain the address, phone number, or employment information of the father listed on the birth certificate. On the enrollment paperwork all of the father's information is filled in with "N/A". The record did not contain any documentation regarding custody of the child.
