Inspector Notes

An unannounced monitoring inspection was conducted on Monday August 8, 2022 to determine the center's compliance with licensing standards. The inspection begin at 11:10 AM and concluded at 1:05 PM. The director was present for the inspection. During the tour, the inspector observed children playing in the gym while the other children were out on a field trip. The inspector observed 7 classrooms, playgrounds, 2 buses, and restrooms. The center's first aid kit and non-medical emergency supplies were inspected and found complete. The required postings were inspected and found to be in compliance. Liability insurance, health inspection, and drills were all in compliance. There were five children's records and nine staff records reviewed. There were violations cited during this inspection.

Based on record review, the center failed to ensure that each child had a physical examination by or under the direction of a physician within 30 days after attendance if not completed prior to attendance. Evidence: 1. There was no physical examination in the record of child #5. The documented first date of attendance for child #5 was 6/21/2022.
